Youth Membership 2020/2021



Please fill out this application in its entirety.
A separate form is needed for each child enrolling.
All necessary signatures must be in place after the youth member clearly understands the rules and regulations of our Youth Department.

MEMBERSHIP
Membership is not required to attend programs sponsored by the Beth Israel Youth Department.
However, it is worthwhile to become a member of the age appropriate youth group.
By paying your membership fees, you will receive:
  • Reduced rate for events.
  • Registration for Kadima and USY members with the International and Regional offices
  • Membership is required to attend Kadima and USY events sponsored by Seaboard Region Youth Department, including regional dances and conventions.
The 2020-2021 dues for Beth Israel Youth are:
(6th – 8th Grades) $50
(9th – 12th Grades) $60
Dues do not roll over to the following year. Membership covers the school year.
For Seaboard Region this means Kamp Kadima/Encampment in August 2020 through events in July 2021.

MEDICAL INFORMATION


MEDICAL RELEASE: 

I hereby grant permission for my child to attend Beth Israel Youth Department events.  In case of a medical emergency, I understand that every effort will be made to contact parent(s), guardian(s), or emergency contact.  In the event I cannot be reached, I hereby give permission to the Physician selected by the persons in charge to hospitalize, secure proper treatment for, and to order injection, anesthesia or surgery for my child named above.  This form may be photocopied for emergency use.

 


PHOTOGRAPHY & VIDEO:
I give my consent for any still or video/computer photographs in which my child may appear to be used for publicity or publications; this includes any still or video/computer photographs that are taken at Beth Israel or at off-site events.


BETH ISRAEL YOUTH ACTIVITIES CODE OF CONDUCT:

I will promote the creation of a religious youth community based on mutual respect and a sense of personal well-being.  I have read the following rules, designed to promote the health and safety of all event participants, and have indicated my unqualified acceptance by my signature and that of my parent/guardian.

  1. Any rules announced by the Youth Director or any Adult Event Leader are to be observed as written rules.
  2. Possession and/or use of any illegal drug, drug paraphernalia, alcohol, cigarettes, or weapons as well as shoplifting/theft of any kind is strictly forbidden.  This rule will be strictly enforced and warnings will not be given.  It will result in immediate expulsion from the current program as well as the next program. Reinstatement requests after the 2nd missed program must be made in writing to the Youth Director and reinstatement is not guaranteed.
  3. Vandalism, disturbing the peace or other inappropriate behavior as determined by the adult leadership will not be tolerated.  Any damage caused will be at the participant’s expense.
  4. No one is to leave the program site at any time without express permission of the Youth Director or Advisor.
  5. All participants will be at all scheduled activities and will arrive on time.
  6. Use of prescription and OTC medications is restricted to the individual for whom they were prescribed.
  7. All participants will be expected to show derech eretz (common courtesy) to one another at all times.  Disrespectful behavior toward staff or other participants will not be tolerated.
  8. While we encourage participants to spend time getting to know each other during events, participants may not be alone in any area unless previously stated by the Youth Director. This is for the safety and security of all participants.
